To use this diary card, click the download button. Print the first page on heavy stock paper, then turn the paper over (according to your printer's instructions) and print the second page on the back of the first.

Alternately, you could print the document on 2 pages, then cut out the pictures and tape or glue them back to back.

I suggest folding the card in half or in thirds so it fits in your purse or pocket.
